Job summary

The Salvation Army in Darwin, NT is seeking a Senior (Complex) Case Manager to lead intensive case work within the Housing Support Program. You will develop person‑centred plans, connect participants to health and community services, and advocate for coordinated care in a trauma‑informed, culturally safe environment.

This maximum term role (to 31 December 2027) offers SCHADS Level 5 pay and works within a multidisciplinary team to support participants facing homelessness and other complex needs.

Qualifications

• Degree in Social Work or related field or equivalent experience with Cert IV in Community Services.
• Experience supporting people with multiple and complex needs including mental health, AOD and trauma.
• Experience completing risk assessments and safety planning.
• Knowledge of Darwin homelessness sector and culturally diverse communities.

Responsibilities

• Provide intensive case management to participants with complex needs.
• Develop individual case plans and coordinate access to health and community services.
• Respond to critical incidents using trauma-informed approaches.
• Maintain accurate records in line with TSA standards.
• Collaborate within a multidisciplinary team to coordinate care.
