Qualifications

• Degree in Social Work, Mental Health, Welfare or Community Development or relevant experience.

Responsibilities

• Provide case management support to Housing Support Program participants.
• Develop case plans reflecting each person’s strengths, needs and self-determined goals.
• Assist participants to attend appointments and engage with community supports.
• Complete intake and assessment, maintain records, and advocate with agencies.
• Conduct outreach case management in private/public housing settings.
