Job summary
The Salvation Army Australia is seeking a Case Manager for the Housing Support Program in Darwin. You will provide outreach case management, develop collaborative case plans, and help participants access community supports and services.
The role requires strong risk assessment, intake, and documentation within a trauma‑informed, inclusive framework. You will work within the Homelessness team, reporting to the Supported Housing Senior Team Leader, and operate under SCHADS Level 4 pay.
Qualifications
- Degree qualification in Social Work, Mental Health, Welfare or Community Development, or relevant experience with a minimum Certificate IV in Community Services
Responsibilities
- Provide case management support to participants in the Housing Support Program
- Develop case plans reflecting each person’s strengths, needs and self‑determined goals
- Assist participants to attend appointments, build capacity and engage with community supports
- Complete intake and assessment and maintain accurate records
- Advocate for participants with government and non‑government agencies
- Support outreach case management in private/public housing
