Placements24 is seeking a dedicated Remote Social Support Coordinator to manage a portfolio of clients from home, coordinating housing, healthcare and community services online. You will conduct needs assessments, develop tailored plans, provide virtual support, and maintain confidential records while liaising with external agencies.

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, or related field.
• At least 3 years of hands-on social work or case management experience.
• Proven ability to work with diverse client populations and manage caseloads independently.
• Strong virtual communication and organization skills; proficient with video conferencing and case management tools.

Responsibilities

• Conduct remote needs assessments using digital tools.
• Develop and implement tailored support plans.
• Coordinate referrals with housing, healthcare, employment, and other services.
• Provide virtual emotional support and crisis intervention as needed.
• Maintain confidential client records and update case progress online.
• Serve as primary contact, ensuring timely communication.
