Job summary

Disability Support Link is seeking experienced Social Workers/Support Coordinators to join our growing team across Melbourne and Regional Victoria. This is a Work from Home contract position offering a 60 to 70 AUD per hour rate.

You will deliver person-centered support coordination, help clients navigate complex systems, and promote independence for people with disabilities. Key responsibilities include assessing needs, coordinating services, providing advocacy, and maintaining compliant

Qualifications

• Tertiary qualification in Social Work, Community Services, Mental Health, Disability Support, or related field.
• Current professional registration or accreditation with relevant regulatory bodies where applicable.
• Minimum 1–2 years' experience in aged care, disability support, community services, or related social services environment.
• Strong knowledge of the National Disability Insurance Scheme (NDIS), relevant rules and legislation.

Responsibilities

• Conduct comprehensive assessments of participants' needs, circumstances, and goals to develop individualised support plans.
• Coordinate and facilitate access to community services, health care, housing, employment, and other relevant support services.
• Provide proactive support, counselling, and advocacy to participants and their families.
• Develop and maintain relationships with service providers, government agencies, and community organizations to ensure seamless service coordination.
• Monitor and review progress against support plan objectives, making adjustments as required.
• Maintain detailed case notes and documentation in compliance with legislation and policies.
• Prepare reports and recommendations for case conferences, tribunal hearings, and other proceedings.
• Provide crisis intervention and emergency support when required.
