Job summary

Precruitment is seeking a Community Care Coordinator to help Elders and community members access respectful, reliable support that enables them to live well and independently. Based in remote Cape York, this role is relationship-focused and requires cultural awareness and sound coordination skills with a strong commitment to community-led service delivery.

You will work closely with clients, families, care workers, service providers and community stakeholders to keep services responsive, well

Qualifications

• Experience in community care, aged care, disability support, or health services.

Responsibilities

• Coordinate services for Elders, people with disability and other eligible community members.
• Work with clients and families to understand needs, goals and service preferences.
• Assist with care plans, service reviews, referrals and follow-up actions.
• Coordinate care workers, schedules and day-to-day service priorities.
• Build effective relationships with health providers, agencies and community organisations.
• Monitor service quality and respond promptly to gaps, concerns and changing needs.
• Maintain accurate client records, service information and program documentation.
• Support reporting, compliance and continuous improvement activities.
