Job summary

Origin Support Services in Cairns region is seeking a dedicated NDIS Support Coordinator to help participants maximise their NDIS plans by linking them with appropriate supports and services. You will work across Cairns, Townsville, and Perth, ensuring plans are implemented with person-centred care.

This role requires proven experience in NDIS Support Coordination or similar, excellent communication, and the ability to build networks with participants, families and providers.

Qualifications

• Mandatory: Proven experience in NDIS Support Coordination or a similar role.
• Current NDIS Worker Screening Check and Working with Children Check.
• Current driver’s licence and access to a vehicle.
• Background in disability, mental health, social work, or community services.
• Relevant qualifications preferred but not essential.
• Strong communication and stakeholder engagement skills.

Responsibilities

• Coordinate and manage services aligned with participants' goals.
• Strengthen participants' capacity to navigate the NDIS.
• Build trusted relationships with participants, families and providers.
• Maintain progress notes and case documentation.
• Promote person-centred planning and participant choice.
