Job summary

Action Justice Well-being in Cairns, Australia, is hiring an NDIS Support Coordinator to work with participants, families and service providers to coordinate supports and deliver plan outcomes.

Qualifications

• Qualification in Social Work, Psychology, Human Services, Mental Health, Community Services, Nursing, or related discipline.
• Demonstrated experience in Support Coordination, Case Management, Mental Health, Psychosocial Recovery Coaching, or Community Services.
• Strong knowledge of the NDIS, mental health systems, and community supports.

Responsibilities

• Assist participants in coordinating their supports and achieving their NDIS plan outcomes
• Provide timely and high-quality support coordination services
• Develop complex service plans to address barriers and implement coordinated solutions
• Liaise with NDIA, NDIS Quality and Safeguards Commission, and other stakeholders
• Monitor budgets, prepare reports, and complete administrative tasks
• Conduct risk assessments, emergency plans, and support plans to a high standard
• Facilitate access to services, including transitions from custody to community
• Advocate for participants’ rights ensuring culturally sensitive and inclusive support
• Maintain accurate documentation using systems such as ShiftCare and PRODA
• Identify service improvements and evaluate marketing opportunities
• Conduct plan reviews and ensure compliance with mandatory reporting requirements
