Pro Care Assist Support Services Pty Ltd in Townsville, QLD seeks an NDIS Support Coordinator / Aged and Disability Case Manager to coordinate disability services for participants under the NDIS. This full‑time role focuses on developing personalised care plans and guiding participants and families through available supports.

Qualifications

• Must have qualification in social work, community services, disability support, or equivalent experience.
• Knowledge of the NDIS principles, participant rights, and regulatory requirements.
• Experience delivering case management, care coordination or support services in disability/aged care sectors.
• Strong interpersonal and written/verbal communication; ability to liaise with multiple stakeholders.

Responsibilities

• Conduct assessments and develop individualised care and support plans.
• Help participants understand and implement their NDIS plans and access funded supports.
• Coordinate delivery of NDIS, disability, aged care and community services.
• Maintain relationships with participants, families, carers, and providers to ensure coordinated support.
• Monitor progress and adjust plans as needed; identify barriers and propose solutions.
• Assist access to community services, healthcare, employment and social activities.
• Provide information, guidance and advocacy on supports and referrals.
• Maintain accurate case notes, plans and progress reports.
• Ensure service quality and safety; uphold professional boundaries and confidentiality.
• Participate in team meetings and ongoing professional development.
