Job summary

Support to Thrive Services Pty Ltd in Hervey Bay seeks an experienced Support Coordinator to assist participants with NDIS plans and linking to services. The successful candidate will build capacity, coordinate supports, and work with families and providers to achieve goals.

The role is permanent part-time (20–25 hours) with potential to move to full-time. A genuine passion for empowering people with a disability and strong communication are essential.

Qualifications

• Have minimum 12 months experience in Support Coordination, or similar experience.
• Excellent communication and problem-solving skills.
• Ability to work independently while also contributing to a team environment.
• A genuine passion for empowering people with a disability.
• Current driver's licence and reliable vehicle (with comprehensive insurance).
• NDIS Worker Screening Check (or willingness to obtain prior to employment).
• Working with Children Check (or willingness to obtain prior to employment).
• Although not essential, relevant qualifications in Community Services, Disability or Individual Support are highly desirable.

Responsibilities

• Support participants to understand and implement their NDIS Plans.
• Connect participants to appropriate supports, service providers and community resources.
• Build participants' capacity to coordinate their own supports and make informed decisions.
• Work collaboratively with participants and their families, carers and other stakeholders.
• Monitor supports and ensure services are meeting participant needs and goals.
• Maintain accurate records and case notes in line with organisational and NDIS requirements.
• Prepare and provide requested reports to the NDIA.
• Identify and respond to risks, barriers or changes in participant circumstances.
