Job summary

Access Support Coordination in Cairns seeks an experienced Support Coordinator to empower participants within the NDIS framework. You will guide clients through options, help them understand plans, and coordinate supports to maximise funded outcomes, while building trustful relationships.

The role requires a background in disability services, a relevant qualification, and current safety clearances. We offer flexible work strategies and strong career growth within a compassionate team.

Qualifications

• Proven experience in a Support Coordination role within the disability sector.
• Qualifications in Disability, Community Services, Social Work or related field.
• Strong knowledge of the NDIS and its processes.
• Able to manage KPIs and billable hour targets effectively.
• Current Driver's Licence, Working with Children Check and NDIS Worker Screening Clearance.

Responsibilities

• Support participants to build meaningful, connected lives beyond their funded supports.
• Help participants understand and get the most out of their NDIS plans.
• Coordinate supports efficiently to maximise outcomes and funding utilisation.
• Gather supporting evidence that reflects participants' evolving needs and future funding requirements.
• Identify opportunities across employment, recreation, education and housing.
• Advocate for participants and liaise with mainstream services to ensure the right supports are in place.
