Job summary

Anglicare Central Queensland Limited in Rockhampton is seeking a dedicated Case Manager for our Homelessness team on a full-time fixed-term basis. You will support people who are homeless or at risk, delivering home-based case management and referrals to housing, health and legal services.

You will develop action plans, assess needs, and advocate to maximise independence and housing options within a trauma-informed framework.

Qualifications

• Certificate IV in Community Services or similar required.
• Experience delivering case management services to vulnerable populations is desirable.
• Ability to work autonomously within a team and align with KPIs.

Responsibilities

• Develop, action and review individual case management plans with participants.
• Provide practical support, budgeting, information, advice and referrals.
• Assess participant needs, including risk assessments and safety planning.
• Deliver proactive, respectful services that maximise independence.
• Advocate for housing access and connection to specialist services.

We arecurrently seeking a Case Manager to join our Homelessness team inRockhampton on a full time basis. They will provide case managementsupport to people who are homeless, at imminent risk ofhomelessness or housed but at risk of homelessness. Case managementis provided to people in their home, in temporary livingsituations, in supported housing, in a community setting or in apublic space.

What will a typical day look like?

• Work closely withparticipants to develop, action and review case managementplans to meet their individual needs.
• Provide practicaland personal support, crisis budgeting, information, advice,and community referrals.
• Provide initial andongoing assessment of participant needs, including riskassessments and safety planning as needed.
• Provide supportthat is proactive, persistent, reliable, practical, andcomprehensive in a manner that is respectful of the dignity andprivacy of individuals.
• Provide services ina way that maximises the participant’s capacity for independentliving.
• Advocate and assistparticipants to access housing and specialist services forexample, drug or alcohol, mental health, legal, domestic, andfamily violence.

MandatoryRequirements:

• Certificate IVqualifications in Community Services or similar.
