Qualifications

  • Experience delivering intervention strategies relating to family support, homelessness, and at‑risk young people.

About the Role

As a Practitioner within our Youth Homelessness Programs, you will provide responsive, person‑centred support to young people who are at risk of or experiencing homelessness across our InSync, Reconnect, and Immediate Housing Response services. You will deliver practical and therapeutic support including case management, crisis and transitional accommodation assistance, mediation and family reconnection, safety planning, counselling, and advocacy. Working collaboratively with young people, families, and external services, you will support pathways to stable housing, strengthen daily living skills, address mental health concerns, and foster safe, sustainable transitions toward independence and long‑term stability.

This is a permanent part‑time position, working 45.6 hours per fortnight (three days per week), based in Cleveland, QLD.

Responsibilities
  • Deliver high‑quality, person‑centred case management, mediation, advocacy and practical support to young people and their families in line with Anglicare's Mental Health and Wellbeing Practice Framework.
  • Work collaboratively with young people and families to identify goals, develop tailored support plans, strengthen living skills, and promote pathways to independence and stable accommodation.
  • Advocate on behalf of young people and families, including coordinating referrals and liaising with specialist services to ensure integrated and effective service delivery.
  • Contribute to program development by participating in strategic planning, identifying service gaps, supporting community‑responsive projects, and assisting with promotional activities.
  • Maintain accurate, timely and high‑quality documentation, reports and case notes, ensuring compliance with organisational policies, funding requirements and legislative standards.
  • Build and maintain strong, cooperative relationships with internal teams, external stakeholders, community agencies and government services.
  • Contribute to a positive team culture by actively participating in meetings, sharing knowledge, supporting colleagues, supervising students (as directed), and engaging in continuous improvement initiatives.
  • Uphold professional standards by maintaining confidentiality, complying with the Code of Conduct and Work Health and Safety requirements, participating in professional development, and undertaking delegated duties in accordance with organisational policies and delegations of authority.
Qualifications, Credentials and Other Requirements
  • Degree in Health, Human Services, Behavioural Science, Social Work, Psychology or similar, and/or equivalent practical experience.
  • Current Queensland Driver Licence
  • Current Blue Card (Working with Children Check)
  • Demonstrated experience providing case management and assessment support to adults and young people experiencing or at risk of homelessness.
  • Experience delivering intervention strategies relating to family support, homelessness, and at‑risk young people.
  • Strong written and verbal communication, negotiation and relationship building skills.
  • Ability to work both independently and collaboratively in a team environment.
  • Competent in Microsoft Office and comfortable learning new systems.
  • Commitment to flexible working hours as required.
