Job summary

RecruitShop is seeking a CHSP Services Coordinator in Spring Hill, QLD to coordinate in-home aged care services for CHSP clients from intake to delivery. This practical role combines client service coordination, administration and communication, working with rostering and care workers.

You will manage onboarding, explain CHSP services to clients and families, maintain records and support with reporting and quality activities.

Qualifications

• Experience in aged care or community services, ideally CHSP, Home Care Packages or Support at Home.
• Understanding of service coordination, client onboarding and basic scheduling.
• Strong organisational and communication skills with attention to documentation accuracy.
• Confidence explaining aged care programs and services to clients and families.

Responsibilities

• Coordinate CHSP services in line with program guidelines and policies.
• Manage client intake and onboarding, including sign-up documentation.
• Explain CHSP and Support at Home services to clients and families.
• Coordinate new services and changes with the rostering team.
• Communicate service requirements to care workers; monitor changing client needs.
• Maintain accurate client records, service notes and compliance docs.
• Assist with reporting, quality activities and general administration.
