Job summary

FOCUS Connect is seeking a Services Coordinator – CHSP to coordinate services from enquiry and referral through to commencement and ongoing delivery in Campbelltown. You will work closely with clients, families, My Aged Care and internal teams to ensure services are delivered efficiently and with the person at the centre.

Ideal candidates will have experience in Home Care or community services, CHSP knowledge, and strong organisational and communication skills to manage high volumes while

Qualifications

• Strong organisation and attention to detail.
• Ability to manage high volumes and competing priorities.
• Experience in Home Care or community services, CHSP knowledge would be highly regarded.

Responsibilities

• Manage CHSP enquiries and referrals from initial contact through to service commencement.
• Complete assessments, Support Plans, reviews and required service documentation.
• Coordinate services reflecting each client’s choices, goals and changing needs.
• Monitor service utilisation and delivery, proactively following up gaps and opportunities for additional support.
• Contribute to achieving contracted CHSP program outputs, helping ensure funded services reach people who need them.
• Maintain accurate records to support DEX reporting, quality, compliance and program performance.
• Build referral relationships and represent FOCUS Connect at community meetings, networks, expos and events.
• Ensure services are delivered in line with CHSP Program Guidelines, Aged Care Quality Standards and the FOCUS Connect Philosophy of Care.
