Job summary

FOCUS Connect is seeking a Services Coordinator – CHSP to coordinate services from enquiry and referral through to commencement and ongoing delivery in Campbelltown. You will work closely with clients, families, My Aged Care and internal teams to ensure services are delivered efficiently and with the person at the centre.

• Manage CHSP enquiries and referrals from initial contact through to service commencement.
• Complete assessments, Support Plans, reviews and required service documentation.
• Coordinate services reflecting each client’s choices, goals and changing needs.
• Monitor service utilisation and delivery, proactively following up gaps and opportunities for additional support.
• Contribute to achieving contracted CHSP program outputs, helping ensure funded services reach people who need them.
• Maintain accurate records to support DEX reporting, quality, compliance and program performance.
• Build referral relationships and represent FOCUS Connect at community meetings, networks, expos and events.
• Ensure services are delivered in line with CHSP Program Guidelines, Aged Care Quality Standards and the FOCUS Connect Philosophy of Care.

We're looking for an organised, proactive and resilient Services Coordinator – CHSP to join our passionate team and help coordinate the delivery of our Commonwealth Home Support Programme services.

This is a fast-paced, high-volume coordination role. You'll manage enquiries and referrals, complete assessments and service documentation, coordinate service commencement and reviews, and monitor services to ensure they remain aligned with each client’s choices, goals and changing needs.

You'll be part of a team that genuinely cares about the people we support and works together to make sure available services reach older people who need them.

About the Role

As our Services Coordinator – CHSP, you'll coordinate services from enquiry and referral through to commencement, review and ongoing service delivery.

Working closely with clients, families, My Aged Care, referral partners and our internal teams, you'll help ensure services are delivered efficiently, accurately and with the person at the centre.

Your responsibilities will include:
