Tender Loving Care Aged Care – Bankstown NSW is seeking an organised CHSP Lead to oversee the client journey from enquiry through onboarding and ongoing coordination.

You will work with clients, families, My Aged Care and referral partners to ensure person-centred service delivery, compliance and continuous improvement.

Qualifikationen

• Experience in aged care, CHSP, or community services and client coordination.
• Understanding CHSP requirements, client documentation and DEX reporting, or willingness to learn.
• Strong communication, organisation and stakeholder management skills.
• Current National Police Check and valid driver’s licence; NDIS screening where required.

• Manage CHSP enquiries, referrals, intake, assessments and client onboarding.
• Coordinate Support Plans, service agreements, reviews and reassessments.
• Monitor service delivery, client needs, utilisation and follow-up actions.
• Maintain accurate client records and documentation, including DEX reporting requirements.
• Support contracted CHSP outputs, referral conversion and program performance targets.
• Build strong relationships with clients, families, My Aged Care and community referral partners.
• Represent TLC Aged Care at community events and referral network meetings.
• Monitor compliance, identify risks and elevate service delivery concerns.
• Improve intake processes, reporting accuracy and operational workflows.

Working closely with clients, families, My Aged Care, referral partners and internal teams, you will support person-centred service delivery, program performance, compliance and continuous improvement.
