Job summary

The Salvation Army is seeking a dedicated Case Manager to join our Supported Accommodation team in Caboolture. You will provide crisis intervention, assessments, and case management to people experiencing or at risk of homelessness, working in a trauma-informed, client-centred framework.

You’ll collaborate with internal teams and external stakeholders, delivering practical support, developing individualised plans, and assisting access to housing and services to improve long-term outcomes.

Qualifications

• Bachelor’s degree or higher in Community Services, Social Work or related field.
• Experience in case management and client assessment.
• Strong understanding of trauma-informed and person-centred practice.

Responsibilities

• Conduct client assessments, intake and triage.
• Respond to enquiries with timely support.
• Develop and implement individualised case plans.
• Provide ongoing case management and advocacy.
• Assist clients to access housing and community resources.
• Complete referrals and safety planning.
• Maintain client records and notes.
• Collaborate with multidisciplinary team.
