Job summary

The Salvation Army Australia is seeking a Case Manager for the Caboolture area to provide phone-based and face-to-face support within community and residential settings. You will work with clients, internal teams and external stakeholders to deliver high-quality services that make a genuine impact.

Key responsibilities include client assessments, developing individualised plans, housing access support and coordinated service delivery.

Qualifications

• Tertiary qualifications in Community Services, Social Work or a related field.
• Demonstrated experience in case management, client assessment and support planning.
• Trauma-informed and person-centred practice.
• Excellent communication and relationship-building skills.
• Ability to manage competing priorities in a fast-paced environment.

Responsibilities

• Conduct client assessments, intake and triage, identifying needs, risks and vulnerabilities.
• Respond to phone, email and walk-in enquiries with timely and effective support.
• Develop and implement individualised case plans with clients.
• Provide ongoing case management and advocacy support.
• Assist clients to access housing, support services and community resources.
• Complete referrals, safety planning and service coordination activities.
• Maintain accurate client records and case notes.
• Participate in case reviews, supervision and local networking opportunities.
• Work collaboratively within a multidisciplinary team to achieve positive client outcomes.
• Contribute to continuous service improvement initiatives.
