Housing Stability Case Manager – Trauma-Informed Support

Job summary

The Salvation Army is seeking a dedicated Case Manager to support individuals and families experiencing homelessness from its Surry Hills base. You will provide trauma-informed, client-centred support, develop tailored care plans, and advocate for housing stability and social connections.

Reporting to the Team Leader, this full-time permanent role requires collab­oration with multidisciplinary teams, attention to documentation, and willingness to travel as needed.

Qualifications

• Diploma or higher qualification in Community Services, Social Work, or a related field (or currently working towards a degree).
• Experience in case management and supporting people experiencing homelessness, mental health challenges, AOD issues, or other complex needs.
• Sound understanding of trauma-informed practice and client-centred approaches.
• Strong stakeholder engagement, communication, and documentation skills.
• Ability to work collaboratively and adapt to changing service needs.
• NSW Working with Children Check.
• Willingness to complete a Nationally Coordinated Criminal History Check.
• Flexibility to travel and work varied hours as required.

Responsibilities

• Deliver case management, crisis intervention, and transitional support in line with the National Homelessness Stream Model of Care.
• Develop, implement, and regularly review tailored case and care plans with clients.
• Support residents and outreach clients through ongoing case management and advocacy.
• Conduct risk assessments and safety planning for outreach and offsite activities.
• Maintain accurate client records, case notes, and reporting requirements.
• Collaborate with multidisciplinary teams and external agencies to achieve positive client outcomes.
• Contribute to a safe, inclusive, and culturally responsive service environment.
