Job summary

The Salvation Army Australia is recruiting an ICMI Case Manager to provide intensive, person-centred support to people experiencing or at risk of homelessness in Leongatha, VIC. This permanent full-time role is on-site and requires a compassionate, skilled professional committed to housing outcomes.

You will develop tailored case and safety plans, advocate for clients, coordinate access to services, and work with a dynamic team to empower clients toward stability and independence.

Qualifications

• A qualification in Social Work, Community Services, Welfare, Community Development, or a related discipline; or equivalent sector experience.
• Experience working with people experiencing homelessness, housing insecurity, or complex support needs.
• Strong interpersonal, advocacy, and case management skills.
• Current Victorian Driver's Licence.
• Current Victorian Employee Working with Children Check.

In this rewarding outreach-based role, you'll provide intensive, person-centred support to individuals with complex needs, helping them secure and sustain safe, affordable housing while reducing their reliance on the homelessness service system. Working alongside clients, you'll empower them to achieve their goals, strengthen community connections, and create lasting positive change.

About the Role

As a Case Manager in Intensive Case Management Initiative (ICMI), you'll work closely with clients through outreach and intensive support sessions, delivering holistic case management that responds to their unique circumstances, vulnerabilities, and aspirations.

You'll develop tailored case and safety plans, coordinate access to specialist services, advocate on behalf of clients, and provide practical support to help people achieve stable housing outcomes.

Reporting to the Team Leader, this is a permanent full-time opportunity, based on-site in Leongatha, VIC.

Conditions

Compensation is in accordance with the SCHADS Award Level 5.

What you will bring

• A qualification in Social Work, Community Services, Welfare, Community Development, or a related discipline; or
• Demonstrated sector experience, knowledge, and capability gained through previous roles, training, and professional development
• Experience working with people experiencing homelessness, housing insecurity, or complex support needs will be highly regardedStrong interpersonal, advocacy, and case management skills
• A genuine commitment to person-centred practice and social justice
• A current Victorian Driver's Licence
• A current Victorian Employee Working with Children Check
