Job summary

The Salvation Army is seeking a compassionate Case Manager to support residents experiencing homelessness at The Open Door and Flagstaff facilities. You will provide person-centred, trauma-informed support, develop tailored case plans, and advocate for housing and services to help clients regain stability.

Reporting to the Team Leader, this fixed-term full-time role is based in Flagstaff. You will work with housing, health, legal and community partners to deliver sustainable outcomes and empower

Qualifications

• Degree qualifications in Social Work, Psychology, Community Services or related discipline.
• Experience in case management within homelessness, housing, or community services.
• Understanding homelessness, its causes and evidence-based interventions.
• Experience building partnerships across health, housing, legal and community sectors.
• Excellent communication and advocacy skills.

Responsibilities

• Deliver high-quality case management and support planning.
• Empower clients to achieve housing and wellbeing goals.
• Advocate for access to housing, health, legal, employment, education and community supports.
• Assist clients to secure medium and long-term housing options.
• Coordinate services with internal teams and external stakeholders.
• Facilitate case conferences and coordinate integrated responses.
• Contribute to service development and continuous improvement.
