The Salvation Army in Flagstaff, VIC is seeking a Case Manager to provide person-centred, strengths-based and trauma-informed support to residents experiencing homelessness and complex life challenges.

You will develop tailored case plans, advocate for needs, coordinate services across housing, health, legal, mental health and community organisations, and work with clients toward stable housing and independence.

Qualificações

• Degree qualification in Social Work, Psychology, Community Services, or a related discipline.
• Experience in case management within homelessness, housing, or community services.
• Strong understanding of homelessness and evidence-based intervention approaches.
• Experience building partnerships across health, housing, legal, and community sectors.
• Excellent communication, relationship-building, and advocacy skills.
• A Current Victorian Driver's Licence
• A Current Employee Working with Children Check

Responsabilidades

• Deliver high-quality case management and support planning
• Empower clients to identify and achieve housing and wellbeing goals
• Advocate for access to housing, health, legal, employment, education, and community supports
• Assist clients to secure medium and long-term housing options
• Work collaboratively with internal teams and external stakeholders
• Facilitate case conferences and coordinate integrated support responses
• Contribute to continuous improvement initiatives and service development
• Support a positive, collaborative, and client-focused team culture
