• Support customer requests and administrative enquiries.
• Coordinate workflow activities across the Care Partnering team.
• Assist with customer reviews, reassessments, referrals, and service mobilisation.
• Support Care Partners and Clinical Care Partners through effective workflow coordination and follow-up.
• Monitor workflow queues and escalations, overdue actions, or customer concerns.
• Maintain accurate customer records and documentation across systems.
• Coordinate actions with suppliers, contractors, and stakeholders.
• Support quality, compliance, and governance processes.
• Contribute to continuous improvement initiatives and new ways of working.
