Job summary

Australian Unity is seeking a Care Partner Support Officer | Aboriginal Home Health in Penrith NSW to coordinate customer requests and workflow, support Care Partners and Clinical Care Partners, and help ensure services are delivered on time. This permanent full-time hybrid role offers an hourly rate of $40.38 under the Care Administration Enterprise Agreement (Level 3), with superannuation.

Qualifications

• Demonstrated commitment to culturally safe practice with Aboriginal and Torres Strait Islander peoples.

Responsibilities

• Support customer requests and administrative enquiries.
• Coordinate workflow activities across the Care Partnering team.
• Assist with customer reviews, reassessments, referrals, and service mobilisation.
• Support Care Partners and Clinical Care Partners through workflow coordination and follow-up.
• Monitor workflow queues and escalate overdue actions or customer concerns.
• Maintain accurate customer records and documentation across systems.
• Coordinate actions with suppliers, contractors, and stakeholders.
• Support quality, governance, and continuous improvement initiatives.
