Job summary

Australian Unity is reimagining health and care delivery in local communities. The Care Partner will support customers through their care journey, coordinating services and developing holistic care plans to help them live independently at home or in chosen settings.

The role is full-time at Hornsby with either permanent or 6-month fixed-term basis. You will manage a customer portfolio, conduct assessments, plan care, and work with clinical teams to ensure safe, person-centred care and ongoing

Qualifications

• Experience in community, aged care, disability or health services.
• Ability to build trusted relationships with customers.
• Excellent communication skills and professionalism.
• Knowledge of funding streams, quality standards, audit requirements.
• Decision-making, problem-solving and time management in fast-paced settings.
• Collaborative mindset across multidisciplinary teams and providers.
• Relevant qualifications in Community Services or related discipline (Certificate IV or Diploma desirable).

Responsibilities

• Build trusted relationships with customers and support them to achieve their goals at home.
• Conduct onboarding assessments and develop holistic care plans.
• Coordinate multidisciplinary services and complete customer reviews.
• Collaborate with clinical teams, families and external providers to deliver person-centred care.
• Support customers through care transitions, hospital discharges and offboarding.
• Identify unmet needs and optimise funding usage to improve outcomes.
• Maintain accurate customer records and contribute to quality and compliance.
• Promote safety and person-centred care across service delivery.
