Australian Unity Limited is seeking a Care Partner to guide customers through their care journey with compassion, coordinating services and developing holistic health plans to age well at home or in preferred settings.

This full-time, hybrid role combines branch-based work with virtual support, collaborating with families, clinical teams and external providers to deliver person-centred care.

Qualifications

• Experience in community or aged care, including case management and coordinating multidisciplinary care at home.
• Strong customer focus with empathy and ability to handle sensitive conversations.
• Excellent verbal and written communication; proficient with MS Office and CRM systems.
• Knowledge of funding streams, quality standards and care transitions.

Responsabilités

• Build trusted relationships with customers and support goals at home.
• Onboard assessments, develop holistic health plans, coordinate multidisciplinary services.
• Collaborate with clinical teams, families and external providers for safe, person-centred care.
• Support care transitions, follow up with discharge planning and changes in health needs.
• Maintain accurate records, participate in audits and quality initiatives.
• Identify opportunities for additional services and growth.
• Promote safety, quality, compliance and continuous improvement.
