Job summary

Australian Unity Limited is seeking a Care Partner to guide customers through their care journey with compassion. You will work with families, clinical teams, and providers to develop holistic health plans and coordinate services, enabling aged care at home or in a preferred setting.

This permanent full-time role offers a flexible hybrid model, 2 days WFH and 3 days in the West Perth office, with a starting hourly rate of 45.56 AUD plus superannuation.

Qualifications

• Experience in community care; knowledge of SAH is essential.
• Strong customer focus with empathetic communication.
• Proficient in Microsoft Office and CRM systems.
• Knowledge of funding streams, quality standards and audits.

Responsibilities

• Build trusted relationships with customers to support goals at home.
• Conduct onboarding assessments and develop holistic health plans.
• Coordinate multidisciplinary services with clinical teams and providers.
• Support care transitions and follow up after discharge.
• Maintain accurate records and participate in audits.
• Identify opportunities for additional services to improve outcomes.
• Promote safety, quality and continuous improvement.
