Job summary

Australian Unity Limited is seeking a Care Partner to guide customers through their care journey with compassion, coordinating services with families, clinical teams and external providers to deliver safe, person-centred care. This full-time, hybrid role can be based from a nearby branch.

You will age well at home or in chosen settings, develop holistic health plans, and ensure safe discharge planning and ongoing support for customers in the community.

Qualifications

• Experience in community or aged care, including case management and coordinating multidisciplinary care in the home.
• Strong customer focus, with the ability to build trusted relationships and handle sensitive conversations with empathy.
• Knowledge of funding streams, quality standards and audits; experience supporting care transitions.

Responsibilities

• Build trusted relationships with customers and support them to achieve their goals at home.
• Conduct onboarding assessments, develop holistic health plans, and coordinate multidisciplinary services.
• Collaborate with clinical teams, families, and external providers to deliver safe, person-centred care and optimal outcomes.
• Support customers through care transitions, including hospital discharge or changes in health needs, and follow up with offboarding where needed.
• Maintain accurate records in enterprise systems, participate in audits, and contribute to quality and continuous improvement initiatives.
• Identify opportunities for additional services and support to enhance customer outcomes and contribute to service growth.
• Promote safety, quality, compliance, and a culture of continuous improvement across all activities.
