Job summary

Australian Unity is seeking a Care Partner to guide customers through their care journey with compassion. You will work with families, clinical teams, and external providers to develop holistic health plans and coordinate services, enabling safe, person-centred care at home or a location of choice.

This permanent full-time role offers flexible hours (8:30am–4:30pm or 9:00am–5:00pm, Mon–Fri) and a hybrid work model with 2 days from home, 3 days in the Clayton office.

Qualifications

• Experience in community or aged care is preferred but not essential.
• Strong customer focus with the ability to build trusted relationships and handle sensitive conversations with empathy.
• Excellent verbal and written communication skills; proficient with technology including MS Office and CRM systems.
• Knowledge of funding streams, quality standards, audits, and care transitions such as discharge planning.

Responsibilities

• Build trusted relationships with customers and support them to achieve their goals at home.
• Conduct onboarding assessments, develop holistic health plans, and coordinate multidisciplinary services.
• Collaborate with clinical teams, families, and external providers to deliver safe, person-centred care and optimal outcomes.
• Support customers through care transitions, including hospital discharge or changes in health needs, and follow up with offboarding where needed.
• Maintain accurate records in enterprise systems, participate in audits, and contribute to quality and continuous improvement initiatives.
• Identify opportunities for additional services and support to enhance customer outcomes and contribute to service growth.
• Promote safety, quality, compliance, and a culture of continuous improvement across all activities.
