Client Centred Home Care is hiring two full-time Care Coordinators on the Gold Coast to keep client schedules aligned with approved supports and to act as the main scheduling contact for the care team. You will work closely with the Complex Health Manager and support workers to deliver consistent, well-coordinated care.

This is a great role for someone who thrives on organisation, loves problem-solving, and enjoys being the go-to person keeping everything running smoothly behind the scenes. You'll play a pivotal role in making sure our clients receive consistent, well-coordinated care, working closely with our Complex Health Manager - RN (CHM), support workers, and the wider care team.
You'll coordinate and maintain client rosters, making sure schedules reflect the supports each client has approved, and that any changes are communicated promptly to clients and support workers. You'll be a day-to-day point of contact for scheduling and service delivery queries, checking in regularly with clients to make sure things are running as they should be.
When something needs a closer look like a rostering gap, a change in circumstances, a support worker concern, or the early stages of an incident, you'll know when to step in and coordinate a response, and when to bring it to the CHM. This role doesn't carry clinical decision-making or case management responsibility, but sound judgement about when to step in and coordinate a response is at the heart of it.
You'll also provide administrative support to the CHM, help keep client documentation and records up to date, and pitch in on recruitment admin like scheduling interviews when needed.

Certificate III or IV in Community Services, or a related field (or equivalent experience)
Experience in a scheduling, rostering, coordination, or administrative support role
Strong organisational, time management and prioritisation skills, with the ability to manage multiple client schedules at once
Excellent communication and interpersonal skills, comfortable liaising professionally with clients, families and support workers
Sound judgement in knowing when a matter needs to be escalated, rather than resolved independently
High attention to detail and accuracy in record-keeping and rostering
Proficiency in Microsoft Office 365 and rostering/scheduling systems, or the ability to pick these up quickly
A collaborative approach and comfort working under direction within a team environment
Experience using a rostering or scheduling system, and/or experience within the Disability, Aged Care or Community Services sector, would be highly regarded.
A Blue Card and NDIS Worker Screening Clearance (or willingness to obtain), and a current Australian Driver's Licence are must-haves for this role.
