Job summary

Warrigal Home Services is seeking a Care Partner to coordinate and manage the care needs of clients living in the community. You will work with the Care Services Manager and broader team to ensure services align with care plans and approved frameworks.

The role requires strong organisation and the ability to manage competing priorities, develop and review care plans, and respond to changing client needs while promoting independence and wellbeing.

Qualifications

• Diploma or Certificate IV in Community Services, Aged Care, Case Management or equivalent tertiary qualification.

Responsibilities

• Undertake comprehensive assessments.
• Develop and review care plans.
• Balance multiple client caseloads while focusing on outcomes and wellbeing.
• Collaborate with clients, families, service providers and internal teams to deliver safe, efficient services.
• Identify opportunities to enhance care delivery and optimise client funding.
