Job summary

Warrigal Home Services is seeking a dedicated Care Coordinator to support older people in the community. You will coordinate and manage client care, undertake comprehensive assessments, and develop and review personalised care plans.

You will work with families, service providers and internal teams to ensure services are delivered safely and on plan, while adapting to changing needs and promoting wellbeing and independence.

Qualifications

• Diploma or Certificate IV in Community Services, Aged Care, Case Management or equivalent tertiary qualification.
• Demonstrated experience in care coordination, case management or related roles within aged, health or disability sectors.
• Advanced assessment and care planning skills including the ability to identify client needs, risks and support requirements.
• Experience working in multidisciplinary care environments.
• Strong interpersonal and communication skills with the ability to build relationships with clients, families and service providers.
• Ability to manage a caseload and prioritise competing demands.
• Demonstrated understanding of person-centred care and wellness reablement principles.
• Demonstrated ability to network, negotiate and advocate.
• Ability to meet Worker screening requirements
• Valid Driver's Licence.

Responsibilities

• Coordinate and manage the care needs of clients living in the community.
• Undertake comprehensive assessments and develop/review care plans.
• Ensure services align with care plans and approved support frameworks.
• Collaborate with clients, families, service providers and internal teams to deliver safe, efficient care.
• Monitor and respond to changing client needs to minimise service disruptions.
• Promote independence and optimal outcomes through proactive care management.
