Job summary

Warrigal invites applications for a Care Coordinator based in Bundanoon, NSW. You will coordinate and manage home care for clients, ensuring services align with care plans and approved frameworks while working with a supportive Home Services team.

You will assess, plan and monitor caseloads, prioritise competing needs, and collaborate with families, providers and internal staff to promote safety, independence and wellbeing in the community.

Qualifications

• Diploma or Certificate IV in Community Services, Aged Care, Case Management or equivalent qualification.
• Experience in care coordination or case management in aged, health or disability sectors.
• Advanced assessment and care planning skills to identify client needs, risks and support requirements.
• Ability to work in a multidisciplinary care environment and communicate effectively with clients, families and service providers.
• Must meet worker screening requirements and hold a valid driver's licence.

Responsibilities

• Undertake comprehensive assessments and develop and review care plans aligned to client goals.
• Balance a caseload and prioritise competing demands in a dynamic environment.
• Collaborate with clients, families, service providers and internal teams to deliver safe, efficient care.
• Support older people to live safely and confidently in their own homes and communities.
