Job summary

Warrigal is seeking a Home Services Care Partner to coordinate and manage care needs for clients in the community across the Southern Highlands and surrounding areas. You will work closely with the Care Services Manager and Rostering teams to develop and review care plans, assess needs, and ensure services align with client goals and approved frameworks, while maintaining safety and wellbeing.

Qualifications

• Diploma or Certificate IV in Community Services, Aged Care, Case Management or equivalent tertiary qualification.
• Demonstrated experience in care coordination, casemanagement or related roles within aged, health or disability sectors.
• Advanced assessment and care planning skills including the ability to identify client needs, risks and support requirements.

Responsibilities

• Coordinate and manage the care needs of clients living in the community.
• Undertake comprehensive assessments and develop and review care plans.
