IWA Australia in Melbourne is seeking a Care Partner to provide client management services under Support at Home. You will deliver person-centred case management and supervise Home Care Employees to support participants with varying needs.

The role involves developing and reviewing budgets, managing risks, and maintaining regular communication with participants and carers to meet care plans, ensuring continuity of care and service quality.

Formación

• Qualified with Diploma of Community Services.
• Experience working with culturally and linguistically diverse communities.
• NDIS clearance, Police clearance and WWCC are required.
• Ability to work at least 4 days per week.
• Ability to speak a second language is desirable.

Responsabilidades

• Assist with referrals to My Aged Care and other supports as needed (NDIS, Access Point, SaH).
• Develop and review quarterly budgets in consultation with the participant and registered supporters/carers.
• Review risk assessments related to the participant home environment as required.
• Communicate regularly with participant and/or registered supporter to achieve care plan goals.
• Allocate and liaise with Home Care Employees to ensure services align with participant needs and goals.
