Job summary

Warrigal Home Services in the Southern Highlands seeks a Care Partner to coordinate and manage community-based care for older clients. You will work with the Care Services Manager, Operations Partner and Home Services team to ensure services align with plans and approved frameworks.

The role requires strong organisational skills, ability to balance multiple clients, and proactive problem-solving to minimise disruptions and promote wellbeing.

Qualifications

• Diploma or Certificate IV in Community Services, Aged Care, Case Management or equivalent tertiary qualification.
• Demonstrated experience in care coordination, case management or related roles within aged, health or disability sectors.
• Advanced assessment and care planning skills including the ability to identify client needs, risks and support requirements.
• Experience working in multidisciplinary care environments.
• Strong interpersonal and communication skills with the ability to build relationships with clients, families and service providers.
• Ability to manage a caseload and prioritise competing demands.
• Demonstrated understanding of person-centred care and wellness reablement principles.
• Demonstrated ability to network, negotiate and advocate.
• Ability to meet Worker screening requirements
• Valid Driver's Licence.

Responsibilities

• Coordinate and manage care needs of clients living in the community to align with care plans and approved frameworks.
• Undertake comprehensive assessments and develop and review care plans.
• Monitor services to respond to changing client needs and maintain positive outcomes.
• Collaborate with clients, families, service providers and internal teams to deliver safe, efficient care.
• Support independence and optimize use of client funding to achieve outcomes.
