Job summary

Uniting AgeWell is seeking a dedicated Care Advisor (Case Manager) for the Barwon Home Care Program in Geelong. You will manage your own client caseload, promote independence, and support clients to live confidently at home. The role is based in Belmont, moving to Waurn Ponds in December, with a 9am–5pm Monday–Friday schedule.

You will oversee initial assessments, care planning, budget management, and coordination with a supportive team to deliver high-quality aged care services.

Qualifications

• Demonstrated skills in Case Management.
• Experience coordinating and providing Aged and Community services.
• Experience in a Clinical or Allied Health setting.
• Desire to deliver outstanding customer service in all interactions.
• Strong communication and interpersonal skills.

Responsibilities

• Performing initial assessments of prospective clients and developing care plans aligned to assessed needs.
• Managing budgets and ensuring services stay within financial constraints.
• Working with Care Organisers to coordinate service delivery per care plan.
• Liaising with referring healthcare providers, allied health professionals, social workers, and community services.
• Facilitating client pathway support through case discussions, conferences, and reviews; handling incident management and complaints.
• Maintaining accurate records of client assessments, care plans, services, and changes.
• Coordinating services within program guidelines.
• Ensuring services meet Aged Care Quality Standards and support audit processes.
