Job summary

Uniting AgeWell invites applications for a Care Advisor (Case Manager) in the West Metro Melbourne Home Care Program. You will manage your own caseload, championing clients’ independence and helping them stay in their own homes with supported services.

You will work from our Braybrook office with a supportive team, delivering a full-time role from 9:00am to 5:00pm, Monday–Friday, contributing to outcomes and community impact.

Qualifications

• Demonstrated skills and experience in Case Management.
• Experience coordinating Aged and Community services; Support at Home experience desirable.
• Experience in Clinical or Allied Health setting.
• Strong communication and interpersonal skills.

Responsibilities

• Perform initial assessments of prospective clients and develop care plans.
• Manage budgets and ensure services are delivered within financial constraints.
• Coordinate delivery of services with Care Organisers as outlined in the care plan.
• Liaise with referring healthcare providers, allied health professionals, social workers and community services.
• Facilitate client pathway support through case discussions, conferences and reviews.
• Maintain accurate records of client assessments, care plans, services provided and changes or incidents.
• Coordinate services within business guidelines and ensure compliance with quality standards.
