Job summary

Uniting AgeWell Limited is seeking a Care Advisor to support older people in Bendigo, delivering person-centred care at home. Based in our Bendigo office, you will manage a caseload, build relationships, and coordinate services to promote independence and wellbeing.

The role is permanent part-time (30 hours/week across four days) with duties including assessments, care planning, referrals, and ongoing client support. Applicants must have relevant certificates and a valid Victorian Driver Licence.

Qualifications

• Experience delivering person-centred, goal-focused support within aged care, health, community services, allied health or disability
• Experience conducting assessments and developing care plans
• Knowledge of Consumer Directed Care principles and person-centred practice
• Strong communication and relationship-building skills with clients, families and health professionals
• Good organisational skills and ability to manage competing priorities while maintaining records
• Understanding of privacy, safety and professional responsibilities in a care environment

Responsibilities

• Build trusted relationships with clients and families through regular contact and support
• Conduct assessments and develop personalised care plans
• Review and adjust care plans as client needs change
• Coordinate services and referrals for seamless, client-focused care
• Support clients in decision-making about services, goals and budgets
• Monitor wellbeing and identify emerging risks or support needs
• Collaborate with health professionals and community services for positive outcomes
• Maintain accurate records and documentation across a diverse caseload
• Respond to client feedback, incidents and concerns professionally
• Deliver exceptional customer service to promote independence at home
