Job summary

Uniting AgeWell Limited is seeking a dedicated Care Advisor to support older Australians to live independently at home. Based in Ballarat, you will manage a caseload, develop personalised care plans and coordinate services with families and health professionals to promote wellbeing, choice and safety.

You will conduct assessments, review plans and build trusting relationships, ensuring accurate records and responsive service delivery.

Qualifications

• Experience managing client caseloads and delivering person-centred, goal-focused support.
• Experience conducting assessments, developing care plans, coordinating services and reviewing client outcomes.
• Strong understanding of Consumer Directed Care principles and person-centred practice.
• Excellent communication and relationship-building skills with clients, families, health professionals and service providers.
• Strong organisational skills and ability to manage competing priorities while maintaining accurate records and documentation.
• Sound understanding of compliance, privacy, safety and professional responsibilities within a care environment.
• Ability to work independently while contributing positively to a collaborative team environment.

Responsibilities

• Building trusted relationships with clients and their families through regular contact and support.
• Conducting assessments and developing personalised care plans that promote independence, wellbeing and choice.
• Reviewing and adjusting care plans as client needs change.
• Coordinating services and referrals to ensure seamless, client-focused care.
• Supporting clients to make informed decisions about their services, goals and budgets.
• Monitoring client wellbeing and identifying emerging risks or support needs.
• Working closely with health professionals and community services to achieve positive outcomes.
• Managing a diverse caseload while maintaining accurate records and documentation.
• Responding to client feedback, incidents and concerns with professionalism and empathy.
• Delivering exceptional customer service while supporting older people to live independently at home.
