Job summary

Uniting AgeWell is hiring a Care Advisor in Bendigo to support older people to live independently at home. You will manage a caseload, develop personalised care plans and coordinate services with families and networks in a community-based setting.

This permanent part-time role (30 hours/week across four days) focuses on wellbeing, choice and independence, with opportunities for training within a values-based, not-for-profit organisation.

Qualifications

• Experience managing client caseloads and delivering person-centred, goal-focused support within aged care, health, community services or disability sectors.
• Ability to assess, develop care plans and coordinate services with a focus on client outcomes.
• Understanding of Consumer Directed Care principles and privacy/safety requirements.

Responsibilities

• Build trusted relationships with clients and families through regular contact.
• Conduct assessments and develop personalised care plans promoting independence.
• Review and adjust plans as needs change and coordinate services and referrals.
• Support clients to make informed decisions about services, goals and budgets.
• Monitor wellbeing and identify emerging risks requiring support.
• Collaborate with health professionals to achieve positive outcomes.
• Maintain accurate records and documentation of caseloads.
