Uniting AgeWell in Geelong is seeking a dedicated Care Advisor (Case Manager) to lead a personal caseload, support older adults to live independently at home, and coordinate services under the Barwon Home Care Program.

Initially based in Belmont with relocation to a brand-new Waurn Ponds office in December, the role offers a full-time schedule, Monday to Friday, 9am–5pm, and the chance to contribute to positive community outcomes.

Qualifications

• Demonstrated skills and experience in Case Management.
• Experience coordinating Aged and Community services; Support at Home desirable.
• Experience and expertise in working in a Clinical or Allied Health setting.
• Commitment to delivering outstanding customer service in all stakeholder interactions.

Responsabilités

• Perform initial assessments and develop care plans.
• Manage budgets and ensure services are delivered within financial constraints.
• Coordinate with Care Organisers to deliver services as outlined in the care plan.
• Liaise with referring healthcare providers, allied health professionals, social workers, and community services.
• Facilitate client pathway support through case discussions, conferences and reviews.
• Maintain accurate records of client assessments, care plans, services provided and changes or incidents.
• Coordinate services within business guidelines.
• Ensure all service delivery meets Aged Care Quality Standards and audits.
