Placements24 is seeking a Remote Elderly Care Specialist to join a virtual team focused on compassionate geriatric support. You will develop personalized care plans, coordinate services with healthcare providers, and connect seniors and families with essential resources.

The role requires a bachelor's degree in a related field and at least 3 years of elderly care experience, with excellent virtual communication, strong organizational skills, and proficiency in video conferencing and EHR systems.

Qualifications

Bachelor's degree in Social Work, Gerontology, Nursing, Psychology, or a related field.
Minimum of 3 years of elderly care or case management experience.
Strong virtual communication and interpersonal skills.
Proficiency with video conferencing tools and EHR systems.
Ability to work independently in a remote setting.
Experience in crisis intervention and problem-solving.

Responsibilities

Develop personalized care plans for elderly clients based on individual needs.
Coordinate services with healthcare providers, specialists, and allied health professionals.
Provide ongoing emotional support and counseling to clients and families via virtual means.
Research and connect clients with community resources, support groups, and social activities.
Monitor client well-being through regular virtual check-ins and assess care plan effectiveness.
Liaise with family members to provide updates and align care strategies.
Assist clients and families in navigating healthcare systems and insurance processes.
Maintain accurate electronic client records and progress notes.
