Job summary

Placements24 is seeking a Remote Care Coordinator to support elderly clients from a home-based setting. You will manage care plans, coordinate with doctors, and liaise with families to ensure high-quality, person-centered care.

The role requires strong communication, organisational, and telehealth proficiency, with a minimum of four years in caregiving or healthcare coordination. A dedicated home office and reliable internet are essential.

Qualifications

• Bachelor's degree in Social Work, Gerontology, Nursing, Public Health, or a related field.
• Minimum of 4 years of experience in caregiving, social work, or healthcare coordination, preferably with an elderly population.
• Proven ability to manage care plans and coordinate services effectively.
• Excellent communication, interpersonal, and problem-solving skills.
• Strong organisational skills and attention to detail.
• Proficiency in using telehealth platforms, video conferencing tools, and digital record-keeping systems.
• A dedicated home office space with a reliable internet connection is mandatory.
• Ability to work independently and manage your time effectively in a remote setting.
• Empathy and a genuine passion for supporting the elderly.

Responsibilities

• Develop and oversee personalised care plans for elderly clients.
• Coordinate appointments with doctors, therapists, and other healthcare professionals.
• Liaise regularly with clients and their families to assess needs and provide updates.
• Arrange for necessary home care services, transportation, and social activities.
• Monitor client well-being and promptly address any concerns or emergencies.
• Maintain accurate and up-to-date client records using our client's proprietary software.
• Advocate for clients' needs and ensure they receive appropriate care and support.
• Manage medication schedules and ensure compliance with treatment plans.
• Collaborate with a remote team of support staff to ensure seamless care delivery.
• Provide emotional support and companionship to clients via remote communication channels.
