Placements24 is seeking a dedicated Remote Case Manager to coordinate care for elderly clients from a fully remote setup. This role serves Gauteng and beyond, leveraging digital platforms to assess needs, create care plans, and liaise with families and providers.

You will work independently, using case management software and EHR systems to document progress, ensure privacy, and advocate for client preferences while maintaining high ethical standards and compliance.

Qualifications

• A relevant degree or diploma in Social Work, Nursing, Gerontology, or related field.
• 3-5 years of case management experience, elderly care preferred.
• Proven ability to manage cases remotely using technology.
• Strong aging-related knowledge and community resources.
• Excellent communication and rapport-building skills online.
• Proficient with EHR and case management software.

Responsibilities

• Conduct comprehensive remote assessments of elderly clients' needs.
• Develop, implement, and monitor individualized care plans with clients and providers.
• Coordinate in-home care, medical appointments, transportation, and social activities.
• Serve as primary contact for clients and families.
• Liaise with healthcare providers, social workers, and community organizations.
• Ensure compliance with regulations and ethical standards.
• Maintain confidential client records via case management software.
• Advocate for clients' needs and preferences.
• Identify risks and implement mitigation strategies.
• Participate in virtual meetings and training.
