Key Responsibilities

• Monitor client well-being and safety through virtual check-ins and, where applicable, remote monitoring technology.

• Advocate for the rights and needs of elderly clients, ensuring their voices are heard and respected.

Requirements

• A relevant degree or diploma in Nursing, Social Work, Gerontology, Allied Health, or a related field.
• A minimum of 5 years of experience working directly with the elderly, preferably in care coordination, case management, or home care settings.
• Strong knowledge of age-related health conditions, social support needs, and geriatric care principles.
