Placements24 is seeking an experienced Senior Care Coordinator to oversee a remote portfolio delivering complex care solutions. You will coordinate services, liaise with healthcare professionals and families, and ensure high-quality, client-centered outcomes from a home office.

The role requires a Bachelor’s in a related field, 5+ years in care management (2+ in a senior capacity), and strong remote communication, analytics, and independence.

Qualifications

• Bachelor's degree in Social Work, Nursing, Gerontology, or related field.
• Minimum 5 years in care management or social work with 2+ years in a senior role.
• Proven ability to manage complex cases and coordinate multi-disciplinary teams.
• Excellent remote communication and interpersonal skills.
• Proficiency with digital case management software and telehealth platforms.
• Strong analytical and problem-solving capabilities.
• Ability to work independently and meet deadlines remotely.
• Deep understanding of community resources and social care services.
• Home office with reliable high-speed internet access.

Responsibilities

• Manage a caseload of clients with complex care needs and ensure coordinated service delivery.
