Job summary

Placements24 is seeking a highly organized and compassionate Elderly Care Coordinator to join a fully remote team. You will develop care plans, coordinate home-based support, manage appointments, and help seniors maintain independence while connecting with families and service providers.

The role requires gerontology or related qualifications and at least 3 years of elder care experience. Strong communication and digital collaboration skills are essential for success in this remote-first

Qualifications

• Qualification in Gerontology, Social Work, Nursing or related field.
• Minimum 3 years in elder care, case management or similar.
• Strong communication, empathy and active listening.
• Proficient with video calls, digital tools and CRM systems.

Responsibilities

• Develop and manage personalized care plans for elderly clients based on their needs and preferences.
• Coordinate a range of services including healthcare appointments, home assistance, transportation, and social activities.
• Serve as a primary point of contact for elderly clients, their families, and external service providers.
• Conduct regular check-ins with clients via phone and video calls to monitor well-being and satisfaction.
• Facilitate referrals to appropriate healthcare professionals, social workers, and community resources.
• Maintain detailed and confidential client records and track service delivery.
• Organize virtual social events and engagement activities to combat isolation among seniors.
• Liaise with healthcare providers to ensure continuity of care and effective communication.
• Stay informed about best practices in elder care, aging well, and relevant legislation.
• Manage scheduling and logistics for remote support services.
