Job summary
Placements24 is seeking a proactive Remote Care Coordinator to support elderly clients from the North West region and beyond. This fully remote role requires coordinating care plans, liaising with families and healthcare providers, and ensuring high-quality, personalized services delivered in clients’ homes where possible.
You will manage client caseloads, document progress in a digital system, and monitor wellbeing while maintaining strict compliance with regulations.
Qualifications
- A relevant qualification in Nursing, Social Work or Gerontology.
- Minimum 3 years in care coordination or similar role.
- Experience with elderly care and remote communication.
- Ability to use digital case management systems.
Responsibilities
- Develop, implement and review individualized care plans for elderly clients.
- Coordinate in-home assistance, medical appointments and social activities.
- Serve as primary contact for clients, families and providers.
- Monitor wellbeing and arrange timely issue resolution.
- Maintain accurate client records in the digital system.
- Liaise with caregivers and service providers to ensure quality.
- Conduct virtual check-ins to gauge satisfaction and evolving needs.
- Ensure compliance with regulations and company policies.
- Facilitate communication among clients, families and care teams.
- Identify opportunities to enhance client experience.
